Please find below the information regarding the School Immunisation Program for Year 7 and Year 10 students.
Consent forms are to be completed by Friday 4th of February to ensure that they are processed in a timely manner prior to the immunisations days at school.
YEAR 7 Students
Students in Year 7 (or age equivalent) receive two vaccinations on the immunisation schedule:
• Gardasil 9 - Human Papillomavirus (HPV), 2 dose course, 6 months apart
• Boostrix – Diphtheria/Tetanus/Pertussis (whooping cough) booster, 1 dose
IMMUNISATION DAYS AT KURNAI CHURCHILL:
17 March 2022: 2 vaccinations - Gardasil 9 (first dose) and Boostrix.
18 October 2022: 1 vaccination - Gardasil 9 (final dose)
IMMUNISATION DAYS AT KURNAI MORWELL:
23 March 2022: 2 vaccinations - Gardasil 9 (first dose) and Boostrix.
26 October 2022: 1 vaccination - Gardasil 9 (final dose)
If you have any questions regarding the school immunisation program, please contact the
Family Services Immunisation Team on (03) 5128 6166.
What you need to do:
• Complete the online consent form by visiting:
Please complete form even if you are taking your child to their doctor
for these vaccinations (tick ‘NO’ on form).
• If you do not have internet access you may collect a hard copy of the form from your
child’s school.
Please complete and submit the consent form by February 4th 2022
Consent remains valid until the full course of vaccinations has been administered. Follow up
reminder letters will be sent if your child is absent/unavailable on the day of immunisations at
their school.
These vaccinations are funded by the Department of Health and are free of charge to all
Victorian students.

• Nimenrix – Meningococcal ACWY
IMMUNISATION DAY AT KURNAI CHURCHILL:
17 March 2022: 1 vaccination – Nimenrix
IMMUNISATION DAY AT KURNAI MORWELL:
23 March 2022: 1 vaccination – Nimenrix
